With personalised, evidence-based care, we guide you through the physical, emotional, and lifestyle changes that may follow cancer treatment, including:
We welcome participants from all over Queensland and northern New South Wales, and you can meet with us in person or via telehealth for your appointments.
Step 1: Referral
Ask your GP or cancer doctor to refer you to the Mater Statewide Cancer Survivorship Service. A doctor or nurse practitioner referral is required to access this service.
Step 2: Eligibility check
Our team reviews your referral and confirms your eligibility.
Step 3: Pre-assessment clinic
If you’re eligible, we’ll contact you to book a nurse-led pre-assessment clinic review. In this appointment, we’ll discuss your medical history and identify any unmet survivorship needs.
Step 4: Doctor’s clinic
You may attend a doctor’s clinic (in person or via telehealth) to discuss your survivorship concerns and receive a personalised survivorship care plan. We’ll also connect you with relevant health services and share recommendations with your local GP.
Step 5: Nurse’s clinic
You may also attend a nurse’s clinic (in person or via telehealth) for health education, referrals, resources, and guidance on accessing services closer to home.
