Chronic disease management plans

Chronic disease management plans are useful for anyone with a chronic medical condition, including cancer, who will benefit from an organised approach to their health care. They may also allow you to receive a Medicare rebate for up to five visits per calendar year to allied health practitioners identified in the plan.
