Register your MFOW event

To register your Mini-Field of Women event ensure you complete the fields below with the correct contact details. These details will be used to contact you if required and as the mailing address for the 'Event in a Box'.

Note: You are only required to complete the fields indicated with an asterisk (*). All information provided is stored securely on the Breast Cancer Network Australia database.

If you belong to a BCNA Member Group please include the group name here.
For delivery of an 'Event in a Box' a street address is required (Note: delivery to a PO Box address is not possible for the courier).

Merchandise
Selling of BCNA merchandise at your event is optional, however if you do wish to sell merchandise please indicate the required quantities below. Merchandise will be shipped on consignment to the address detailed above.
Product information: $5.00 each pin, bags contain 20 pins and will be sent out on consignment

Product information: $15.00 per pack, 2 pairs of socks per pack (1 x white and 1 x navy) Packs will be sent out on consignment.

Promotion and publication
As the coordinator of this event you understand that a Mini-Field of Women is a BCNA initiative and will be presented as such. You agree to present the Mini-Field of Women on behalf of Breast Cancer Network Australia and that any photographs and videos you provide to us can be used in future BCNA projects and publications.
